Medical Records Request

You have the right to view or get copies of your or your child’s medical record. You can view them online via our patient portal or make a request to have the records mailed, faxed or emailed. You can also request your health information be sent to any person or entity.

OPTION 1:

Your records are available 24/7 through PatientConnect, our online portal.

PatientConnect gives you greater access and convenience.

  • View your medical notes from your health care provider.
  • View your radiology and pathology results.
  • View your upcoming appointments.
  • Communicate with your doctor’s office by sending a secure message.

OPTION 2:

Powell Valley Healthcare has partnered with RecordQuest to provide the safest and fastest electronic release of medical records. Effective April 15, 2024, Powell Valley Healthcare patients and requestors will use RecordQuest’s portal to request medical records. All requestors will be directed to this online site.

OPTION 3:

Use the following form to transfer your requested health information to another provider or facility, to another third party, or for yourself. We accept non-PVHC forms if they comply with federal and state law.

You may also choose to write a letter requesting records, it must include:

  • The patient’s signature
  • Identity of the person to receive the record
  • Where to send the medical record
  • Specifics of what records are being requested (timeline ex “clinic visits, last 3 years”

To have records from other facilities added to your medical records here, contact the facility where you received care and have the copies sent to PVHC.  We will then add them to our electronic health record system.

Processing Your Request

Records will be available within ten business days. If using the online request system you should have them in 24-48 hours.

If your record is not available within ten days, we will let you know, and your record will be ready within thirty days from your original request.

If the information requested does not exist or can’t be found, we will let you know within 10 business days.

Medical records can be provided by mail, fax or email. Only in emergency situations can records be picked up as requests are not processed here. A valid photo ID is required.

Charges for Copies

There is no charge for records sent directly to a physician, or those requested by a patient.

Requests made by parties other than a patient may be charged a fee consistent with state law.

Your Rights

You may revoke release authorization at any time by submitting a revocation form to your health care provider, unless the provider has already acted on the original request.
